Acknowledgment of Service and Company Status
GC IV Concierge LLC acts as an administrative and technology facilitator that connects clients with licensed medical professionals[cite: 45]. By using our services, you acknowledge that clinical care is provided by independent licensed practitioners and not by the business entity itself[cite: 12]. All clinical decisions and treatment plans are governed by our Medical Director as required by F.S. § 400.9935[cite: 18].
Non-Emergency Agreement
OUR SERVICES ARE NOT FOR MEDICAL EMERGENCIES. If you are experiencing chest pain, difficulty breathing, severe trauma, or any life-threatening condition, you must immediately call 911 or proceed to the nearest emergency room[cite: 44]. We do not provide acute emergency interventions[cite: 44].
Medical Disclaimers and FDA Compliance
IV therapy is provided for elective wellness and nutritional support[cite: 46]. You acknowledge that our IV fluids, vitamins, and customized infusions have not been evaluated by the FDA for the prevention, treatment, or cure of specific diseases[cite: 46]. Our services are not a substitute for regular primary care or specialist medical consultations[cite: 46].
Establishment of Treatment Relationship
To comply with Florida Board of Medicine standards, a 'Good Faith Exam' (GFE) must be conducted via synchronous video telehealth before any treatment is initiated[cite: 24, 26]. You agree to provide a full and accurate medical history, including all current medications and allergies, during this clinical encounter[cite: 27]. We cannot establish a treatment relationship through audio-only phone calls or online forms alone[cite: 29].
Mobile Service Logistics and Entry
We provide services in private residences, offices, and hotels across the Florida Gulf Coast and East Coast, including Miami-Dade, Broward, Palm Beach, and Martin counties. You agree to provide a safe, sanitary, and well-lit environment for our nursing staff. We reserve the right to perform a 'Second Screening' at the door and may refuse service if contagious illnesses are present or if the treatment environment is deemed unsafe.
Cancellation and Refund Policy
Due to the specialized nature of mobile medical dispatch and the sterile compounding of IV solutions, we maintain a strict cancellation policy[cite: 51]. Once a practitioner has been dispatched or a custom IV bag has been prepared, fees are non-refundable[cite: 51].
No Insurance and Payment
GC IV Concierge LLC is a cash-only wellness service[cite: 8, 48]. We do not accept or bill any third-party insurance carriers, Medicare, or Medicaid[cite: 48]. By booking a service, you accept sole financial responsibility for the direct cost of treatment[cite: 48].
Governing Law and Dispute Resolution
These terms shall be governed by and construed in accordance with the laws of the State of Florida[cite: 3]. Any legal actions or disputes arising from our services shall be resolved through binding arbitration in the South Florida jurisdiction where the company's principal place of business is located[cite: 79].
